Blood draws are completed at a Quest Diagnostics Patient Service Center of your choosing. Follow the steps below to schedule your appointment.

Please note, if you currently have a Quest account, log out of your account and do NOT log in before scheduling this appointment
1. Schedule Your Appointment
a. Visit the Quest website to schedule your appointment: https://www.questdiagnostics.com/
b. Select your cookies option according to your preference. You can “accept all cookies”, “reject cookies”, or choose specific cookie settings via “cookie settings”:
c. Scroll down, and click “Find a location”.
d. Set "Test Needed" to "All Other Tests." Enter your zip code. Adjust "Radius" as desired. Select "Search."
e. Select a location containing "Quest Diagnostics" or "Sonora Quest" in the name by clicking the green "Schedule Appointment" button in that lab's box. If no locations with "Quest Diagnostics" appear, please contact 1-888-299-2364 or help@adni4.org
f. A new window/tab may open. Click "Schedule appointment" again.
g. A "sign in" window will pop up. Click "Continue as a guest" at the bottom.
h. The top of the screen shows the current month and clickable days. Select a date to see available morning and afternoon appointments. If you do not see "Morning (AM)" and "Afternoon (PM),", no appointments are available that day, so you will need to click other days. Click the "v" arrow next to the month to see different months, or the "<" or ">" arrows to see different weeks.
NOTE: Please only schedule Monday-Friday. If you schedule on the weekend, you will be turned away.
i. Click on an available time (grey times are no available) that works for you. To help you plan, please note the study requests that you fast (no food or drink except water) during the 6 hours before your appointment.
j. Once you select a time, a green "Continue" button will appear on the right (or on the bottom on mobile). Select "Continue."
k. Complete the "Who's visiting?" form. For the question: "Would you like to provide insurance for the person being tested?" Select "The person being tested will bring it to the appointment." Please note, you do not actually need to bring insurance: study costs are covered and will not be linked to your insurance.
l. Once all details are provided, a green "Continue" button will appear. Select "Continue.
m. Review the appointment details. You can select "Edit" next to a section if needed. Otherwise, select "Schedule Appointment."
n. The final page will show details of your appointment.
Alternatively, you can call Quest’s automated scheduling line at 1-888-277-8772. They will ask if your test requires specialized processing, and you can reply “no.”
2. Bring Your Order Confirmation Sheet
You will need to show your Order Confirmation sheet to Quest staff once you arrive at your blood draw appointment. Please bring a printed copy, if possible. To protect your privacy, your real name, date of birth, etc. are not on this sheet or linked to your blood sample. Instead, we use unique codes.
Where can I find my order confirmation sheet?
There are three ways to find your order confirmation sheet: in your email, in your participant portal or requesting to have it mailed to you.
1. Email: After you joined the Blood Biomarker study (by agreeing to the consent form on the ADNI study website) you should have received an email from our study team with the Order Confirmation Information sheet attached as a pdf, which you can view by clicking on it.
a. In the email from our study team with the Order Confirmation Information sheet, you can select “Order Confirmation Information Sheet”.
b. Click on “Order Confirmation Information Sheet” to view and print your sheet.
c. Here is an example of what the two-page Order Confirmation looks like.
2. Participant Portal: You can also view and/or print your Order Confirmation Information sheet by signing into your participant study portal.
3. Want a copy mailed to you? Call 1-888-299-ADNI (2364) or email info@adni4.org and share your mailing address. If requesting a mailed copy, do not schedule your Quest blood draw appointment until you have received the printed paperwork.
